Visitor Exception Approval Request Procedure and Form

(This does not apply to hospital/clinic patient visitors or clinical care locations.)

To support limiting the spread of the COVID-19 virus, UAMS currently has a no visitor policy in place.  All activities that may be supported via virtual delivery are strongly encouraged to be completed this way.

In recognition that some operations may not translate well to virtual delivery and propose external visitors on campus, limited exceptions to this policy may be considered to support institutional mission and goals.  This process is used for visitors that are requesting entry for a one-time purpose and do not fall under the purview of the Academic Visitor Policy. Examples include prospective student visit, interview (faculty, staff, or student), external speaker for conference/seminar, meeting attendance.

To request an exception for approval of an external visitor, please submit the required information below at least one week in advance of the visit.  All visitors must complete the UAMS health screening process when arriving on site.  Visitors should be instructed to park in Parking 2, and enter at the Parking 2B screening entrance.  All visitors must have a scheduled time of arrival appointment time.  Visitors must be met at the screening station by the designated Visitor Escort.  The Visitor Escort must remain with the visitor at all times on campus to ensure that no unapproved areas are visited.

A schedule report of approved visitors will be on file with the screening station each day to coordinate approved visits.

For approved visits, please ensure the visitor receives instructions regarding:

  • Where to arrive and park (Parking 2)
  • Where to enter the building (Parking 2B – take the elevator or stairs down one floor to this entrance)
  • Required COVID-19 health screening questions below and acknowledgement that if any questions are positive or if the visitor has temperature of greater than 100.4, they will not be able to enter.

The COVID-19 screening questions (as of July 30, 2020) include:

  1. Have you traveled outside Arkansas in the last 14 days?
  2. In the last 14 days, have you been in direct contact with someone who is confirmed to have COVID-19?
  3. Do you have a new onset cough?
  4. Do you have a sore throat?
  5. Do you have shortness of breath?
  6. Have you had a fever greater than 100.4 degrees or chills in the last 24 hours?
  7. Do you have new onset muscle ache, not explained by exercise or activity?
  8. Do you have loss of taste or smell?
  9. Have you been tested for COVID-19 in the last 30 days?

This process does not apply to hospital/clinic patient visitors or clinical care locations.

  • Please list all areas that are being requested to be visited. Clinical care locations are not allowed. An exception for faculty interviews may be requested for clinical areas.
  • Name of visitor for badge * Required
  • This person is responsible for meeting the visitor at the Parking 2B screening station at the appointment time and escorting them at all times on campus.
